    How is the third insert placed in the stone? If the insert is rotated the hole must be longer than the three together.

    The only one of those I've seen up close, was in a museum, the 3rd one didn't play a part of the lifting it just kept the other two separated in their cuts

    To expand on what Frank S said, the center section wasn't made like what is shown in the video. It was a few thinner pieces cut square to drop in the hole to secure the tapered parts.
