Really not a lot can be said about this, almost anyone's natural reaction to try and prevent something of this nature from falling goes against thoughts of safety. Hope she wasn't injured too badly.
Could it have been prevented? Possibly and almost probably but the driver misjudged the point which he should have turned the pallet jack sideways on the lift gate. The lift gate also looked like it was sloping down even before the weight was on it so it may not have been in 100% serviceable condition having possibly leaked down a little or part of the structure was worn. As for the person on the ground she should have not tried to stop the falling pallet in the first place, any reputable carrier will have load insurance, once she put her hands on the side of the pallet, she gave the insurance company a possible out. They would question the driver and he through advice of council would explain when he saw the person on the ground move in front of the pallet out of his vision fearing for her safety may have momentary caused him to become distracted causing him to loose control of the pallet jack, or since he couldn't see her, he hesitated in lowering the hydraulics on the pallet jack to settle the pallet on the lift gate not knowing where her hands might be.
There are a lot of things that can go wrong when unloading a trailer and more than a few of those happened in this instance.
