Did all steam locomotoves operate in that manner, or just some of them?

The idea behind geared locomotives is to drive as many wheels as possible to improve the tractive effort. This makes them attractive for use in difficult terrain as in logging and mining.

There are three classes of geared locos...

    SHAY

Two or three cylinder vertical engine driving longitudinal side gear shaft. Boiler offset to side opposite engine.

    HEISLER

Two cylinders in a V-configuration under centralized boiler. Engines drive a centrally mounted driveshaft that is geared to wheels.

    CLIMAX

Similar to Heisler but engines point forward at an angle of 45 deg to vertical.
