Improper sling length. it looks to be a 30° sling angle. at that angle each of the 4 slings would be required to lift the equivalent of half the total weight of the load

Looking at the video in detail. it doesn't look like the cables snapped, but rather, the hooks slipped off:


That is the first cable to come loose and the hook is intact. That was the right side forward cable. Next to fail was right side rear and then the object rolled and the rest failed.

I've lifted with hooks that way but I've never walked underneath it when it was raised. I've seen the hooks slip before and even dropped the load. That memory makes me extra careful because I know I'm not fast enough to get out of the way when it is falling...

That makes sense as well. Never hook over the eye always hook through the eye

Why trust a hook at all. It should have been eye shackles.

They only had the hook over the edge of the flange. Very bad, I'm surprised it didn't fail sooner.

The crane operator should have know better. Poor training all around.

Any idea where this happened? Perhaps what make of truck?
